Who Receives Home Health Care Service?

Anyone who is home-bound has orders from a physician for home health services (preferably a Primary Care Physician – PCP) and has insurance coverage. Any client who does not require institutional care and for whom outpatient clinical care is not adequate can receive care in the comfort of their home. Our agency will provide skilled nursing and social work services. We will also provide physical, speech, and occupational therapists and any other services requested by your physician.

How Is Service Provided?

When the agency is contacted and a physician’s order is obtained, an initial assessment is completed by a registered nurse to determine the individual’s home health care needs. Further physician’s orders are requested to provide treatment. Physicians, discharge planners, private insurance companies, clients, community facilities, other public health service providers, and families may refer patients.
