Though Go Faith Medical Services, Inc. Dba Faith Medical Services was founded in 2004, the Home Health Agency became Medicare & Medicaid certified in 2005. Faith Medical Services received initial Accreditation by CHAP in 2012 and have continued to maintain CHAP (complying with CHAP standards of excellence) Accreditation ever since. We are a full-service home health agency dedicated to meeting all its patients' needs. We believe that patients can reach optimal health levels in the comfort of their own homes with assistance from care professionals working in conjunction with the patients' primary care physician.

Our Pledge

To provide a collaborative team approach to patient care in the comfort and familiar environment of the patient's home, to ensure continual quality care during the post-hospital phase, to provide all medical needs throughout the wellness process, to be your medical liaison and coordinate care between the hospital and the physician, to provide qualified nursing and other home health services to meet each patient's individualized needs, and to present a patient-centered care to give value to our patients’ ideas, opinions, and decisions.
